I just stumbled across this opener that I've been using lately. Here it is:

"Do you know any good jokes?"

That's it. Works like a charm.

a) it's simple and pretty much impossible to screw up the delivery

b) it works for everyone, from little kids to old ladies, singles or groups. Almost everyone knows at least one joke.

c) It's playful and immediately gets her qualifying herself by trying to think of something funny.

d) It sets you up to neg her afterwards. "Really? That's really the best joke you know?" etc

Stripper: Hey, honey, what's your name?
PUA: [look skeptical] What's yours?
Her: Autumn
PUA: Oh. I'm Barack Obama.
Her: What?
PUA: Barack Obama. [Look Away]
Her: . . . ?
PUA: [Look at her and smile] Is Autumn your real name?
Her: No.
PUA: Alright then. [Look away]
Her: Well, ok, actually my name is Vanessa.
PUA: [Look back and smile] Ah, well my name is J, pleasure to meet you. See, now we can talk like civilized people.

I haven't field tested this a whole lot, but when I have it's worked 100% of the time

The other day I was walking up to a cafe, and I reached the door at the same time as a HB. Now I already knew this girl; she used to work at a restaurant in town, and I'd gotten to know her a bit. I was in a relationship at the time, though, and never pursued her. I'm single now, thus I was happy to see her.

We stood at the door and talked for a bit. I've always gotten a pretty good, flirty vibe off of her, and this convo was no exception. It went well for a couple minutes, then, when the conversation hit a natural pause, she opened the door and walked into the cafe
